Description of services
Clients may receive one or more of the following services as part of my service. Hypnotherapy, administration services, telephone consultations. In addition clients may receive mp3 downloads, emails and letters. Bespoke personalised work and hypnotic interventions both before and during the booked session time.
Price
The price of the service you have agreed is given either in your confirmation email, via messaging services or by telephone discussion. This breaks down the proportion of the service that is covered by a deposit and the remainder that is due on your first session. If further sessions are agreed you will be notified in full of the cost of an hourly price.
Right to Cancel
Under the regulations you have 14 days from entering into my service contract to cancel it. However please note before I work with you I will agree with you that my service starts straightaway. This is because preparation work for your sessions commences before we even meet. In this instance you will still have the right to cancel but you must pay for the value of the service that is provided for you up the point at which you cancel. This sum is usually the cost of the deposit. If I provide my service in full within the 14 days the right to cancel will be lost.
Commitment
As a client your commitment starts from the moment that the deposit is taken and extends to the end of your session or the program that we have agreed in advance. If you agree further sessions or programs the commitment is extended again from the point at which a further deposit is taken to the end of the agreed session or programme
Payment
The balance of the treatment (minus the deposit) must be paid by direct deposit into the therapist’s bank account 24 hours before commencement of the first session, or in cash on commencement of the first session. The cost of the session plan is non-refundable, and clients must be aware that, failure to complete the sessions will render the agreement void and will reduce the overall effectiveness of the plan, with no full or partial refund.
Cancellation of appointments
If for any reason the therapist is unable to fulfil an appointment, 2 working days notice will be given to the client, and the therapist will fit the client into another mutually agreeable date.
Cancellation of appointments with less than 2 working days notice (48 hours) for any session (working days meaning 9:00am to 17:00pm Monday to Friday) will result in the client losing that session. The client must take full responsibility for an incomplete programme if this occurs.
Appointment times
Contact between our sessions will be limited to telephone or email. The client must not turn up unannounced at the practice site. Failure to comply will result in the immediate cessation of treatment without refund.
It is the client’s responsibility to note down the appointment times and to arrive at their sessions on time. If the client fails to turn up at the allotted time the appointment will not run over the time given agreed and the client will lose that time. If the client arrives more than twenty minutes late, the therapist reserves the right to cancel the appointment altogether. There will be no replacement appointment in these circumstances and the client takes full responsibility for an incomplete programme.
Hypno-band
All weight loss programmes must be completed within a three month period. Failure to complete the course within this time frame due to cancelled appointments by the client means that the client will lose any appointments not yet completed.
The cost of the hypno-band session treatment plan is offered at the price agreed for one two hour session (session one ) and four further one hour sessions. If it is deemed that further analysis for dealing with emotional and psychological issues is required, this will be offered to the client at the normal hourly rate. However, the client is under no obligation to undergo further services. If the client chooses to participate in hypnotherapy for psychological and /or emotional issues, the dates of the hypno-band treatment will be changed to accommodate. Likewise if the client requires exterior counselling that is not provided by the therapist, or the services of a nutritionist, the dates will be changed to accommodate the extra sessions. However, the hypno-band treatment will be delayed for no more than a period of three months. Failure to book in dates after the three month period has elapsed means that the client will lose the fee they have paid and will no longer have the option to book in for the hypno-band treatment.
Anti-social behaviour
Any anti-social behaviour by the client will result in the immediate cessation of treatment.
Case history
A full case history will be taken from the client on the first session and the client will agree to answer all questions in full, to the best of their knowledge and as truthfully as possible.
Confidentiality
The therapist will ensure that the confidentiality of the client will be maintained in all but the most exceptional circumstances and all information collected during the sessions will be protected at all times. Information will only be disclosed under a Court Order (civil, criminal or coroner’s Court) or where not to disclose would cause danger or serious harm to others. Information may also be shared with an NHS medical practitioner or other Health Professional, but only with the client’s agreement.
Disclosure of medical conditions
- The client agrees that they do not suffer from any of the following before undergoing any form of Hypnotherapy:
- an under or over active thyroid which has not been medically stabilised (GP consent to hypnotherapy will be required)
- Bulimia or anorexia
- Severe IBS
- A genetic or physiological reason for your weight gain
- Are insulin dependant
- Bi-polar disorder
- Schizophrenia
- Psychosis
You should seek the advice of a qualified medical practitioner before commencing any treatment or if you have any questions related to your mental or physical health, physical fitness, or medical conditions. Also, it is not recommended that you stop or alter any treatment you are currently receiving without prior consent of your doctor or mental health advisor.
Governing body/complaints/confidentiality
I am a member of The Complementary and Natural Healthcare Council (CNHC) in the UK which is a voluntary regulator for complementary therapists, set up with government support to protect the public. I abide strictly by the codes and ethics of both organisations and hold professional indemnity Insurance. Clients have access to the complaints and discipline procedures of the appropriate professional body at any time.
GDPR
I am fully compliant with General Data Protection Legislation (“GDPR”) (EU) 2018 and any national implementing laws, regulations, secondary legislation, as and when amended or updated in the UK , as well as any changes to legislation to the GDPR or the Data Protection Act 1998 that come into force after the aforementioned date.
Tasking requirements
Where asked, clients must endeavour to carry out tasking requirements before/in between /after sessions as part of the treatment plan. Not undertaking them could reduce the effectiveness in treatment. This includes being fully aware of the information provided to you on the website about the Hypno-band procedure before the first session begins.
I will be checking myself daily for COVID-19 symptoms. This will include taking my temperature each day. If I become aware of any COVID-19 symptoms I will immediately cancel any booked appointments and follow government guidance about self-isolation.
I will also be checking daily if anyone in my household is experiencing any COVID-19 symptoms, which could include temperature checks. If anyone in my household experiences COVID-19 symptoms I will cancel all appointments and put myself into quarantine for 14 days, as per government advice. Please note that if I have to self-isolate I will re-schedule your appointments until such time as it is safe to see you. All clients that have existing paid appointments will get priority on dates as soon as I come out of isolation and I will not take on any new clients until I have fulfilled the paid appointments. To this end I will not be offering refunds following my own self isolation.
No Shows
No shows are treated the same as previously, you will lose the paid appointment if you do not let me know that you cannot attend. The exception to this is if you are unable to contact me due to hospitalisation of yourself or any other exceptional circumstances, I would deem appropriate.
These guidelines will also be sent to every client as part of the booking in email.
Therefore it will be assumed that by paying either your deposit or full payment for your sessions you have read and agreed to these terms.
Confidentiality and NHS Track and Trace
My existing terms and conditions refer to the limits of confidentiality relating to the balance of public interest. Public interest is the general welfare and rights of the public that should be recognised, protected and advanced. Disclosures in the public interest, based on the common law, are made where this is essential to prevent a serious and imminent threat to public health, national security, the life of the individual or a third party, or to prevent or detect serious crime.
This clause may become relevant if I contract the virus and am obliged to inform the NHS of people I’ve been in contact with. In this case, I may need to share your name and contact details but not the context in which I know you. However, you may be contacted by the NHS. Therefore, confidentiality will be broken if necessary for reasons of public interest in the area of public health.
Every client will receive an email advising them of this clause. If you explicitly DO NOT want any information shared with the NHS please respond in writing to this email explicitly requesting this.
